Adult Day Health Services (ADH)


ADH is a 3 to 6 hour a day program running Monday through Friday. Our program promotes optimal functionality based on an individual’s concerns, which are identified in the care plan. Individuals are placed into one of two groups:


Offering opportunities for meaningful participation in community activities. Establishing roles of leadership and partnership within one’s community. Participants benefit from group training and/or activities of daily living, communication and socialization, which include:

• Social, recreational, and physical activities

• Field trips and other outings

• Nutritious meals and snacks

• Transportation

• Caregiver support


May include but not limited to:

• Behavioral needs requiring a behavioral support plan

• Participants include those with ICAP scores of 34-70

• Health concerns that require monitoring, intervention, and supervision (such as specialized skin care, positioning and uncontrolled seizures, etc.)

• Includes wheelchair participants.
